Attention Methuen Residents!!

Preschool applications for Peer Role Models in Methuen Public Schools Integrated Preschool classes will be available at all grammar schools:

February 27th, 2012 through March 5th, 2012

Applications will be returned to the Special Education Office only, and will be accepted from:

Monday, February 27th until 3:00 p.m. on Monday, March 5th

This is a tuition-based program.  An application fee of $50.00 is also required (unless your family income falls within the two lowest income brackets; in that case the application fee is $15.00.  Income will need to be verified by providing most current tax return.)

Message From the School Nurse      

Dear Parent,

In order to comply with the laws of the Commonwealth of Massachusetts and Methuen Public Schools policy, every student entering kindergarten must show proof of having received the following immunizations:

5 DPT's

4 POLIO's

2 MMR's

1 VARICELLA

3 HEPATITAS B's

LEAD TESTING

The only exceptions to this policy are medical and/or religious contradictions.

If a child has had chicken pox and he/she is immune against the disease and does not require this vaccine. A physician's certificate attesting to these facts must be brought to the school before enrollment.

It is required that your child be given a physical examination which includes a vision screening with stereopsi according to the MA protocols before entering kindergarten. We request that this is done by your family doctor or at a local clinic an be recorded on the accompanying physical exam form.

Documentation of lead screening is also mandatory prior to entering kindergarten. Lead screening is available at 305 Essex St., 2nd floor, Lawrence, MA from 9:00AM to 12:00PM and 1:00PM to 3:30PM. No appointment is necessary but a phone call is preferred. The telephone number is 978-681-4940.
